How do I know if my dog’s ACL repair has failed?
Signs that the repair may have failed include a sudden return of significant lameness after a period of clear improvement, complete refusal to bear weight after previously doing so, or a new swelling event around the joint. These are not normal fluctuations. Contact your surgeon promptly. Radiographs will confirm whether the implant or repair is intact. Do not wait to see if it improves on its own.
Is TPLO recovery faster than extracapsular repair recovery?

Can my dog sleep in bed with me after ACL surgery?
Not safely during the early recovery period. Jumping onto or off a bed is exactly the kind of sudden, uncontrolled load that can break a repair before it is healed. Both of you will be exhausted and missing each other, but dogs should sleep on ground level on a supportive surface for the first 6 to 8 weeks. For many dogs this means a crate at night. Set up a comfortable sleeping space next to them if you want to be close. You probably will.
My dog seems fully recovered at 6 weeks. Can I let them run?
No. Looking fully recovered at 6 weeks is extremely common after TPLO and TTA, and it is one of the leading causes of repair failure. The bone is still healing. The only way to confirm that healing is sufficient is a radiograph, which is why the 6 to 8 week recheck exists. Continue the programme until your surgeon clears the next stage, regardless of how convincingly your dog has been performing the “I am obviously fine” routine.
Will my dog need surgery on the other leg too?
Possibly. Cornell University’s orthopaedic team says more than half of dogs that rupture one cruciate will rupture the other within approximately one year. ACVS explains that cruciate disease in dogs is degenerative, meaning both ligaments are typically in a similar state of deterioration. Weight management and ongoing joint health monitoring are the most practical tools for delaying or reducing the severity of a second rupture. It is not inevitable. It is just worth knowing about.
How can I keep my dog calm during ACL surgery recovery?
Crate or small-room confinement when unsupervised. Frozen puzzle toys and stuffed Kongs for mental engagement without movement. Scatter feeding and nose work to use the brain without loading the limb. Carpet runners on slippery floors. And for dogs with significant confinement anxiety, ask your vet about trazodone or gabapentin, both commonly used during restricted recovery periods. Sometimes keeping the dog calm requires medication. That is not failure. That is management.
Is arthritis inevitable after dog ACL surgery?
Yes, some degree of osteoarthritis in the affected joint is expected over time, even after successful surgery. ACVS, VCA, and BluePearl all note this clearly. Surgery significantly slows progression, reduces pain, and improves function compared to no treatment. The goal is a comfortable, functional dog who can live a full and active life. Weight management and appropriate exercise after recovery are the most impactful ongoing tools for keeping that life as good as possible for as long as possible.
